Join us and update your PRC file. Please bring your tribal ID and any updated insurance information. It is mandatory to annually update!
Coverage will be suspended by Dec 20, 2024, if PRC has not received your update.

Human Services
Human Services will have their 2025 Application to fill out and will accept with the following documentation:
- Tribal ID
- Driver License/State ID
- Proof of income
